If you get divorced, and the Final Judgment of your divorce awards your ex-spouse a portion of your Pension benefits, your benefits will be affected and you must provide a Qualified Domestic Relations Order.

You will have to send proof of age for you, and if you are married, proof of age for your spouse and a marriage certificate.

To change your address, you must complete and sign the change of address form and submit it to the Trust Fund Office.
