What documents do I need to send with my application?
You will have to send proof of age for you and your spouse.
Member Proof of Age:
You must submit proof of your age in order for us to calculate your entitlement to benefits. Please see the next page for acceptable proofs of age.
If you are married:
In addition to your proof of age, you must submit copies of the following:
- Spouse’s Birth Certificate (see next page for acceptable proofs of age)
- Marriage Certificate
If you are Divorced:
In addition to your proof of age you must submit copies of the following:
- A Filed copy of your Divorce Decree, including all pages.
- A Qualified Domestic Relations Order
If you are Single, Widowed or have been Married for less than one year:
In addition to your proof of age, you must submit copies of the following:
Beneficiary’s Birth Certificate (see next page for acceptable proofs of age)
- Beneficiary’s Social Security Card
- Spouse’s Death Certificate
How do I update my address?
To change your address, you must complete and sign the change of address form and submit it to the Trust Fund Office.
What is the earliest date I can retire?
Age 55.
